The scaphoid usually rotates into flexion, and the lunate rotates into extension. This is apparent on plain static radiographs. Between December 1994 and December 1996, 26 patients with a scapholunate ligament injury (three with SLD 1. degrees, 16 with SLD 2. degrees and 7 with SLD 3. degrees ) underwent an operation for dorsal capsulodesis in the Klinik für Handchirurgie, Bad Neustadt/Saale.

The patient was subsequently treated with corticosteroid injection of the wrist which only provided relief for less than a week. Other treatments including home care and over‐the‐counter pain medication did not provide relief. Scapholunate dissociation is one of the most common forms of carpal instability, which is often overlooked and leads to associated morbidity. Early clinical and radiological presentation may be subtle; hence an understanding of wrist biomechanics is vital in early diagnosis and treatment.
